Getting Started
Below is the link for Dominique’s HIPAA compliant client record keeping system.
- The website will ask you to create a profile and will walk you through completing most of the forms.
- All forms must be signed with your current legal name.
- The only form you will need to individually select is either the Adult or Adolescent Intake Form, depending on which one you need. This form is located under the “Forms” tab, once your profile has been established.
- Each member of a couple or family is required to complete their own profile and individually sign all forms.
